The original Dr. Mario certainly left its mark in gaming, taking its place alongside other similar puzzle games such as Tetris and Puyo Puyo, not to mention the main character’s regular appearances in the Smash series. Naturally it deserves some stage representation as well.
I think the natural choice would be to set a stage within the giant pill bottle the game takes place in. The players would fight inside the bottle, with some red, yellow, and blue blocks making up platforms throughout. It would be a bit of an officially sanctioned “boom box,” common in Smash’s stage maker, with only one exit at the top, although particularly strong attacks will be capable of shattering a hole in the bottle’s side.
